Pillars Academy Enrollment Paperwork - Our enrollment packet is a mix of school specific queries and paperwork required by the state. 

CAL LIC 700 Authorized Pickup - As required by California Law, anyone picking up a child must be submitted in writing from the child's parent. 

CAL LIC 701 Physician's Report - As required by California Law, to be filled out by the child's primary care provider before s/he is allowed to attend school.
